An extensive clinical and laboratory study is being carried out to determine the effect of chronic intoxication with trinitrotoluene (TNT) on the population of human neutrophils. The study was begun during 1975 when a contamination of the water supply at a TNT plant in Lake City, Washington with TNT and/or 2,4-dinitrotoluene was reported. One year later an increased incidence of patients with suspected septicemia was reported at the nearby Veterans Administration Medical Center. Twenty patients with such an illness were studied. In addition to Gram-negative rods, most patients had neutrophils in their blood that demonstrated an increase in the nitroblue tetrazolium (NBT) reduction test. The study has continued over the past 3 years. The laboratory investigation has been expanded to include evaluation of the peripheral blood of the patients with septicemia and controls. Recently, the amount of trinitroglycerin and the capacity of granulocyte phagocytosis were also assessed in a few of these patients. Subsequently, four other patients were evaluated, with two having demonstrated the same toxicologic reaction as the 20 patients previously reported. Fourteen patients with suspected septicemia have been evaluated. The NBT test was found to be positive in the blood of 12 of these patients. Patients with positive NBT tests have included men and women of all ages, blacks and whites, alcoholics and non-alcoholics. Some patients, notably, the three adult male cases have been described in an abstract previously. A small number of patients have been treated with antimicrobial drugs, including penicillin and metronidazole, but the treatment was not successful. Seven of the patients have been treated with corticosteroids, including prednisone and dexamethasone, and five of these patients had some improvement.[Cimetidine (Tagamet) in the treatment of duodenal ulcer (author's transl)]. As a result of a controlled and randomized study conducted by the Diatra Company, it has been established that an oral dose of cimetidine 1 g every 8 hours is therapeutically as effective as 20 mg/kg of antacids in the treatment of duodenal ulcer. These results have been confirmed by the systematic and statistical analysis of clinical studies performed on patients treated with cimetidine and/or antacids during the year 1980.
